Incident Responder Report

If your fire company or ambulance squad responds to an incident in the New Jersey Turnpike or Garden State Parkway right of way, the NJTA asks that you fill out a report and submit it as soon as possible. You can do so using the forms below.

IMPORTANT

This report must be completed in its entirety and submitted as soon as possible after responding to a fire call within the New Jersey Turnpike or Garden State Parkway right-of-way.
